Release And Waiver Of Liability Agreement

I acknowledge that my child has voluntarily chosen to participate in Master the Regents’ review class. By mailing in a check to Master The Regents, LLC and having my child attend the review day,

I am allowing my child to participate in the activities with the knowledge of the potential danger involved, and agree to assume any and all risks of bodily injury, death, or property damage whether those risks are known or unknown. I hereby release Master The Regents, its officers, employees, or agents from any liability, costs and damages resulting in my child’s participation.

I also give my consent for Master the Regents to seek emergency treatment for my child if necessary, and I agree to accept financial responsibility for the costs related to this emergency treatment.
